Clearly set goals and the vision for change
Every project, including implementing change, must have a clear goal. Set out the vision you have when embarking on the change, what is involved (i.e. scope of the project), how you will evaluate if and when the change has happened and whether it has been successful.
Involve your team in developing plans and strategy
Do not make decisions about your team in an "about us without us" style. Subordinates will certainly have numerous practical suggestions, so involve them in developing plans and strategies around change.
Expect resistance to change and be prepared to deal with it
All change brings fears and has its opponents. This always applies - even if it is objectively a change for the better. In short, people often prefer the status quo. Expect resistance, accept it as a necessary evil and prepare a strategy in advance for how you will deal with it.
Make changes gradually
If possible, make changes gradually. Much better than a sudden 180° turn is to implement changes incrementally and gradually.
After implementation, evaluate the process and make any final adjustments.
After you have implemented the change, evaluate the project. Tell yourself what could be improved next time and, if necessary, make final changes to the project you have just completed so that the result is as close to the ideal as possible.
